Asthalin® Respirator Solution is indicated for the routine management of chronic bronchospasmunresponsive to the treatment with conventional inhalers. It is used to treat coughing, wheezing, shortness of breath and other breathing problems caused by chronic obstructive pulmonary disease (COPD) and asthma. Asthalin® Respirator Solution is recommended to use in patients over 4 years of age.

Description
Asthalin® Respirator Solution is manufactured by Cipla and belongs to the family of medicines known as adrenergic bronchodilators, which help to open up the bronchial tubes (air passages) in the lungs. They relieve cough, wheezing, shortness of breath, and troubled breathing by increasing the flow of air through the bronchial tubes. Its active ingredient Salbutamol, a short-acting, selective beta2-adrenergic receptor agonist, that can increase airflow to the lungs by relaxing the muscles in the airways. Asthalin® Respirator Solution should be applied using a face mask or mouthpiece with a nebulizer that changes the solution into a fine mist to inhale. By inhaling the medication, the medicine has a direct action in the lungs where it is needed most, that allows reducing the risk of side effects.

MOA (mechanism of action):
Salbutamol stimulates beta2-adrenergic receptors which are predominant receptors in bronchial smooth muscle of the lung. The medicine stimulation beta-2 receptors which leads to the activation of enzyme adenyl cyclase that form cyclic AMP (adenosine-mono-phosphate) from ATP (adenosine-tri-phosphate). The high level of cyclic AMP relaxes bronchial smooth muscle and decreases airway resistance by lowering the concentration of intracellular ionic calcium in the airways. This also inhibits the release of bronchoconstrictor mediators such as histamine, leukotreine from the mast cells in the airway. Salbutamol relaxes the smooth muscles of airways, from trachea to terminal bronchioles, thus facilitating breathing.
Dosage
Asthalin® Respirator Solution is for inhalation use only, to be inhaled in through the mouth via a suitable nebulizer, as instructed by a physician. The solution should not be injected or swallowed. It may be administered intermittently or continuously.
Intermittent Administration
Adults:
- 0.5 ml of the solution(2.5 mg of salbutamol) diluted to a final volume of 2 ml with sterile normal saline;
- 1 ml (5 mg of salbutamol) diluted to a final volume of 2.5 ml as a maximum dose.
The solution should be inhaled through a nebulizer for about 10 minutes until the aerosol is gone.
- 2 ml (10 mg of salbutamol) of the undiluted solution.
The respirator solution should be inhaled through a nebulizer and the patient to inhale the nebulized solution until bronchodilation is achieved, usually from 3 to 5 minutes.
- 2 ml (10mg of salbutamol) of the undiluted solution.
In this case, the undiluted solution is inhaled through a nebulizer until the aerosol is gone.
Children (4 years and older):
- 0.5 ml (2.5 mg of salbutamol), diluted to 2-2.5 ml with sterile normal saline;
- 1 ml (5 mg of salbutamol) as a maximum dose.
The Intermittent administration may be repeated up to four times daily.
Continuous Administration
Adults: 1-2 ml solution made up to 100 ml with diluents inhaled through a nebulizer. The usual rate of administration is 1-2 mg per hour.
Children (6 - 12 years): the use and dosage should be prescribed by a doctor individually.
How to use Asthalin® Respirator Solution in the nebulizer:
1. Take one container of Asthalin® Respirator Solution or mix the exact amount of it using the dropper provided for each dose.
2. Place the inhalation solution in the medicine reservoir or nebulizer cup on the machine.
3. Connect the nebulizer to the face mask or mouthpiece.
4. Use the face mask or mouthpiece to breathe in the medicine.
5. Use the nebulizer for about 5 to 15 minutes, or until the medicine in the nebulizer cup is gone.
6. Clean all the parts of the nebulizer after each use.
Missing a dose
If you miss a dose you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not exceed the doctor’s recommended dose.
Overdose
If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.
Storage
Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. The contents should be used within one month after the bottle is opened. Throw away any unused medicine after the expiration date. Keep out of the reach of children.
